If the client has received any help that they can think of from the State of Michigan, it can be listed here. If they cannot think of anything, or they are unsure, it can be left blank.
What if the client is Homeless?
If the client does not have a permanent address, they still must use an address to complete the application for mailing purposes. Often a Shelter will allow the use of their address. A trusted friend or family member’s address can also be used. If your organization is comfortable with the use of your address, you can use yours to receive DHHS mail on behalf of the client.
Can I leave an answer blank?
Yes. With the exception of the client’s name and address, fields can be left blank. There are some fields that won’t be relevant to some applicants. It is important, however, that the most information possible is being submitted to DHHS. Although the application can be submitted with limited information, DHHS still needs a lot of information to be able to determine benefits. Include as much information as possible on the application.
Why is the phone number important?
When a SNAP application is submitted to DHHS the caseworker must call the client to do a phone interview. If this step is missed, and the caseworker does not reach the client, the case will eventually be closed without a benefit.
