It is important to remember that the information we use for our services is sensitive and incredibly important to the welfare of our clients. This information is to be kept confidential in accordance with the terms of the contracts governing our relationships with Elder Law of Michigan, Inc. (ELM) and Michigan’s Department of Health and Human Services (DHHS). Confidential information includes any personally identifiable information. Personally identifiable information is any information collected, used, processed, stored, or generated as part of a screening or application, and can include but is not limited to social security number, government issued identification number(s), date of birth, address, telephone number, email address, or the individual’s name in combination with any of the other information listed.
Personally identifiable information shall be used for no other purpose than screening the client for potential eligibility and/or applying for the programs that we provide application assistance with and that the client agrees to apply for or be screened for. At no time will ELM or its Network Partners use, sell, rent, transfer, distribute, or other disclose or make available this information for any purpose outside of those described above or in other contracts between ELM, DHHS, and/or our network partners.
Any information gathered during the screening or application process will be used ONLY for the purpose of applying for benefits on behalf of the client, with the client’s or their authorized representative’s consent. ELM may also elect to use this information to send follow up and redetermination reminders to the client and/or Network Partners for those aforementioned benefits, as well as provide information about other services provided by ELM.
